We would like to inform you that in our clinics for organizational reasons  

Vaccination against Covid-19 WILL NOT be carried out. 

 

Below are useful websites, vaccination points and telephone numbers closest to our clinics. 

 

You can register for vaccination:

  • by calling the National Immunization Program free hotline:  989

  • by contacting the selected vaccination point - list below

  • by sending an SMS to the number 664 908 556 with content: we vaccinate

  • via e-registration available on the home page of the patient.gov.pl.

To sign up for vaccination via patient.gov.pl, you need to log in with a trusted profile or an ID card with an electronic layer (e-document).

People who have an online account with cooperative banks, PKO BP or Pekao SA can log in to this form with the same data that they use to log in to their iPKO or Inteligo online account with PKO BP, online account with Pekao SA or a cooperative bank.

From July 1, you can get a second dose of the COVID-19 vaccine at any vaccination point. 

Below are instructions on how to easily change your second dose location. You can do it:

  • via the free 989 hotline - the consultant will ask for your PESEL number and the city or address of the new vaccination point,

  • by logging into the Patient's Online Account at  www.patient.gov.pl - there, in the COVID-19 vaccination tab, you can click "Change visit" and choose any vaccination point.

 

Remember that if you want to take a second dose at the same point, you do not need to make any changes. All you have to do is report at the appointed time and take the second dose.
