How to sign up?

 

Declarations  can be downloaded in a PDF file, fill in your details (1. strona declaration), sign the rest and we will help you to complete the Registration declaration. To be able to use our services under the National Health Fund, you must complete the declarations dla:

1. DOCTOR

2. NURSES

3. midwife(for women, beboys up to 2 months of age)

Declarations can be obtained from our Registration to be completed at home

or 

you can fill them out directly in our clinic - all you need is an ID card.

Changes to the declaration of choice can be made free of charge up to two times a year - each subsequent change involves a payment of PLN 80 to the account of the Provincial Branch of the National Health Fund.
